Skin substitutes, with the 2026 payment rules in plain English.

PCSS sources skin substitutes (cellular and tissue-based products) for practices that apply them in the office. Medicare changed how it pays for them on January 1, 2026 — we'll walk your team through what applies to your setting and your MAC.

Which skin substitutes does PCSS carry?

What does Medicare pay for skin substitutes in 2026?

In the office setting, Medicare pays skin substitutes as incident-to supplies at a national rate of $127.14 per square centimeter before geographic adjustment. Actual payment varies by locality, and payment is not a coverage decision.

Did the new skin substitute LCDs take effect?

No. The DFU/VLU skin substitute LCDs scheduled for January 1, 2026 were withdrawn on December 24, 2025. Some MACs still have older LCDs in effect.

Who buys the product?

Your practice. Because Medicare pays the practice for the product as an incident-to supply, skin substitutes go through our wholesale model on net-30 terms.
