How dressing codes are split

Most surgical dressing codes come in three pad-size bands — 16 sq. in. or less, more than 16 up to 48 sq. in. and more than 48 sq. in. — with separate codes for dressings with and without an adhesive border. Fillers are billed per unit: per 6 inches for rope and other fillers, per gram for dry collagen filler, and per fluid ounce for hydrogel gel. Bandages and roll gauze are billed per yard. The descriptors are in the DME MAC Supplier Manual's Appendix A.

Pad-size examples

Pad area decides the band. For alginate and other fiber gelling dressings:

  • A 4×4 pad is 16 sq. in., so it falls in the first band: A6196.
  • A 6×6 pad is 36 sq. in.: A6197.
  • A 10×10 pad is 100 sq. in.: A6198.
  • Alginate rope is a filler, billed per 6 inches: A6199.

The same bands apply to foam (A6209–A6211 without a border, A6212–A6214 with one), collagen (A6021–A6023) and most other wound covers.

Rules that trip claims

Collagen codes need a PDAC listing

Medicare allows collagen dressing codes A6021–A6024 only for products with a coding verification published on the PDAC Product Classification List. Check the listing for the specific product before you dispense it.

Wound cleanser isn't a covered dressing

Medicare does not cover wound cleansers (A6260) under the surgical dressing benefit, including when they come in a kit.

Dressings applied during a billed service aren't billed separately

When a practitioner applies dressings as part of a professional service billed to Medicare, the dressings are included in that service and are not separately payable. Dressings sent home with the patient may be billed, with the patient's home — not the office — as the place of service.

Silver and honey: coded by the main material

The LCD lists silver and honey among materials whose safety and effectiveness “have not been established.” Multi-component dressings are classified by their clinically predominant component, and the impregnated-gauze codes aren't used for gauze that contains silver or honey. PCSS lists these products by their base dressing type — confirm the code for the specific item.

Confirm before you bill

These are typical categories by product type and pad size, for reference. The code for a specific product depends on its PDAC classification and the documentation behind the claim. Confirm final coding with your billing staff — or ask us: every PCSS product page lists suggested codes size by size.

For CMS's official wording of any code from A6000 to A6599, use the wound care HCPCS code lookup.
